Best Google Ads Agency for Addiction Treatment: How to Choose.

Treatment center ads run under some of Google's strictest rules, and the conversion that matters happens on the phone. Here is how to judge an agency on certification, search, call handling, privacy and admissions.

The best Google Ads agency for addiction treatment centers confirms your LegitScript and Google certification before any campaign is built, writes keywords and ads around levels of care, insurance and location, and measures results as admissions tracked through your own admissions line, with patient information kept out of the ad platforms.

This guide covers certification, search terms, call handling, tracking and the questions to ask. For how we approach the whole category, see our marketing for addiction treatment centers page, and for how we run search, our Google Ads management page.

What a specialist agency does that a generalist does not.

A generalist treats a treatment center like any lead generation account: broad keywords, every call counted as a conversion, remarketing lists. Here those choices can stop ads from running, waste money or put patient privacy at risk. A specialist starts from certification, builds search around how people and families look for care, and treats the admission as the conversion.

LegitScript and Google certification.

Google restricts ads for recovery oriented drug and alcohol addiction services. Its examples include clinical treatment providers, sober living homes and mutual help groups, lead generators or referral agencies, and crisis hotlines. Services for behavioral addiction, impulse control disorders or nicotine addiction are outside the policy. At the time of writing, approved advertisers can run these ads in Australia, Canada, France, Ireland, New Zealand and the United States.

  • Two approvals, in order. In the US and Canada, Google requires certification through LegitScript's addiction treatment program, and the advertiser must also be certified with Google through its healthcare advertising application, submitted per location or group of locations.
  • Not every provider qualifies. Google says not all providers are eligible for LegitScript certification. LegitScript lists lead generators, call centers or marketers paid to refer patients, and sober living homes without licensed clinical services among those that may not qualify.
  • Advertising standards carry over. LegitScript's standards cover accurate website content and truthful advertising, and it says affiliation with a lead generator can lead to denial. Running addiction ads on Google, Bing or Facebook without certification may also be grounds for denial.
  • Fees and monitoring. LegitScript charges a per facility application fee and an annual certification fee, published on its site, and certification is kept through ongoing monitoring.

Search terms, negatives and Performance Max.

Search is where certified centers usually start, because searchers are already looking for help. A specialist organizes campaigns by intent:

  • Level of care. Detox, residential, partial hospitalization, intensive outpatient and medication assisted treatment each get their own ad groups and landing pages, matched to the care you actually provide.
  • Insurance and payment. Searches that name an insurer or ask about cost go to an insurance page and a verification of benefits path.
  • Location and urgency. City and near me searches, and urgent searches that should reach a person on the admissions line rather than a form.
  • Families. People searching for a loved one need different ads and a page written for them.
  • Brand. Your own name, protected so callers reach your line and not a directory or impersonator.

Negatives matter as much. Common waste includes physical therapy and injury rehabilitation searches, job searches, nicotine and vaping searches outside the policy, and levels of care or locations you do not offer. Lists should come from your own search terms reports.

Performance Max is a weaker fit here than in most categories. Google does not allow advertiser curated audiences such as Customer Match or your own data segments for ads in sensitive health categories, so the usual audience signals are limited, and automated campaigns can drift toward low quality calls. If an agency runs it, ask for brand exclusions and a qualified admissions goal. Local Services Ads cover a defined list of categories; check Google's current list before planning around them.

Admissions calls, verification of benefits and tracking.

In treatment, Google Ads mostly produces phone calls. The agency's work does not stop at the click:

  • Call handling. Calls should reach trained admissions staff at any hour, or an answering service the center controls. A missed call at night can be a lost admission.
  • Verification of benefits. The form should ask only what admissions needs, route to a covered system and say when someone will respond.
  • HIPAA aware tracking. HHS has published guidance on tracking technologies on health websites, part of which a federal court vacated in June 2024. What remains still says tracking vendors that receive protected health information need a business associate agreement. Use call tracking on HIPAA eligible plans, keep health details out of ad and analytics tools, and have counsel approve what leaves your systems.
  • Admissions as the conversion. Track the path from inquiry to verification of benefits, clinical recommendation, admission or referral out. Then send qualified outcomes back to Google through offline conversion imports, without health details, so bidding learns from admissions and not from every call.

Criteria to judge a Google Ads agency for treatment centers.

How to evaluate a Google Ads agency for an addiction treatment center
What to checkWhat a strong agency shows youRed flag
CertificationYour LegitScript and Google certification status checked before any build, with the site prepared for reviewOffers to launch before certification or to run through someone else's account
KeywordsCampaigns by level of care, insurance, location and family intentOne broad rehab campaign for every service
NegativesLists built from your search terms reportsA copied negative list nobody has updated
Call handlingA plan for answering every call, including nights and weekends, on your own lineCalls routed to a third party call center
Tracking and privacyHIPAA eligible call tracking, agreements in place and no health details sent to ad platformsPixels on intake forms and remarketing based on treatment interest
Conversion goalQualified inquiries and admissions imported from your admissions systemEvery call over 30 seconds counted as a win
ReportingCost per admission by source and level of careClicks, impressions and call counts only

Questions to ask in the first call.

  • How do you confirm our LegitScript and Google certification, and what do you change on our site before review?
  • Which search terms would you build around for our levels of care, and which would you exclude?
  • Where do our calls go after hours, and who answers them?
  • Which call tracking and form tools do you use, and are they covered by agreements for health data?
  • How will admissions from our system get back into Google Ads without health details?
  • What does your monthly report show, and how do you calculate cost per admission?

Then run the selection in this order:

Confirm certification.
Know your LegitScript and Google status before you talk to agencies. It sets your launch date.
Pull your numbers.
Gather last quarter's spend, calls, verifications, admissions and admissions by source from your admissions system.
Shortlist two or three agencies.
Favor agencies whose own pages cover certification, call handling and privacy in this category.
Run the same first call.
Ask each one the questions above and compare the answers side by side.
Get a written plan.
Ask for the compliance check, tracking setup, campaign structure and reporting in writing before you sign.

Red flags.

  • Offers to sell you leads or calls, or to pay or take a fee per patient referred. Federal law at 18 U.S.C. 220 makes it a crime to pay or receive remuneration for referrals to recovery homes and clinical treatment facilities, and many states have their own patient brokering rules. Ask your counsel which apply to you.
  • Claims about locations, services, accreditations or success rates you cannot support.
  • Retargeting based on interest in treatment, or pixels that capture form fields and insurance details.
  • Reporting that never reaches admissions.
  • Account access you only get on request.

How pricing usually works.

  • Flat monthly management fee. A set fee for defined campaigns, separate from ad spend. Predictable, and it does not reward spending more.
  • Percentage of ad spend. The fee rises with budget, which can reward bigger budgets over better admissions.
  • Setup or project fees. For tracking, certification preparation and landing pages.
  • Pay per call or pay per admission. Be careful. Paying per patient can raise patient brokering questions, so have counsel review any fee tied to referrals.

Ad spend is paid to Google separately. LegitScript fees are paid to LegitScript.

Where Theory Road fits.

Theory Road runs Google Ads for addiction treatment centers, including centers pursuing LegitScript approval, as described on our addiction treatment centers page. Paid campaigns on Google and Meta launch only after LegitScript certification is approved; we prepare the site and marketing for review and run organic and local search while certification is pending. Our Google Ads management work starts with an audit of conversion actions, goals, negatives, landing pages and CRM handoff, then builds search campaigns by service, location and urgency, with negatives drawn from search terms reports.

  • Admissions first. We set up call tracking on HIPAA eligible plans with numbers by source, CRM stages for inquiry, verification of benefits, recommendation and admit or referral out, and server side signals that send qualified inquiries to ad platforms without health details.
  • Privacy. Patient information stays out of ad and analytics platforms, tools that touch patient data are covered by agreements, and we do not build retargeting based on treatment interest.
  • No lead buying. We do not buy leads or pay for referrals, and calls go to your admissions staff or an answering service you control, never an aggregator.
  • Reporting. Admissions by source and level of care, cost per admission, qualified inquiry rate, inquiry to admit rate and speed to live answer, reviewed monthly.

What is the best Google Ads agency for addiction treatment centers?

The best Google Ads agency for an addiction treatment center confirms LegitScript and Google certification first, builds search around levels of care, insurance, location and families, keeps patient information out of ad platforms, and reports cost per admission from your admissions system. Theory Road works this way for treatment centers; judge any agency, including us, against those criteria.

Do treatment centers need LegitScript certification to run Google Ads?

In the US and Canada, yes, at the time of writing. Google requires LegitScript addiction treatment certification and Google's own healthcare certification for recovery oriented drug and alcohol addiction services.

Can an agency run our ads while certification is pending?

Not for addiction services. Ads for these services need approval first. Organic search, your Google Business Profile, your website and your tracking can all be built while you wait.

Can we use remarketing or Customer Match?

Google does not allow advertiser curated audiences such as Customer Match for ads in sensitive health categories. Plan around search intent instead.

What should we measure?

Admissions by source and level of care, cost per admission, qualified call share, inquiry to admit rate and speed to a live answer.
